Your Specific Responsibilities:
This is a highly responsible professional position that serves as the Minority Health Liaison with the Florida Department of Health in Hendry and Glades Counties (DOH-H/G) and reports to the Senior Human Services Program Specialist - SES. This position is responsible for promoting and coordinating activities to advance health equity and improve health outcomes for residents through developing partnerships, project planning, and implementation of projects that address social determinants of health (SDOH).
Researches, compiles, analyzes, and shares data on health equity, health disparities, and SDOH with internal and external groups. This includes information related to health disparities within Hendry and Glades Counties, barriers to achieving health equity, best practices to address SDOH, and available funding opportunities.
Promotes the development of programs that improve access to health care services. Facilitates a health equity coalition that includes organizations that serve the community. Engages partners in developing and updating health equity plans. Works with partners to coordinate and implement activities from health equity plans. Conducts presentations and provides updates at coalition meetings. Prepares agenda items and meeting minutes.
Develops and submits program workplans, health equity plans, quarterly reports, and other documents as required, within established deadlines. Monitors and evaluates progress on health equity plan objectives.
Coordinates technical assistance trainings and provides resources that provide better information dissemination, education about health disparities, improve cultural competency, and increase the quality of health outcomes and delivery of health care services. Coordinates and participates in local community outreach events to promote health equity initiatives, share approved messaging, and engage the community.
Assesses training needs of DOH – H/G staff and other community partners to improve their capacity for addressing social determinants of health; provides and/or coordinates the provision of staff training related to health equity.
Attends required trainings and meetings, including DOH Southwest Region Minority Health team and DOH Office of Minority Health and Health Equity meetings.
Participates on the DOH – H/G Performance Management Council (PMC) and attends regular meetings and assists with program budgets as needed.
Performs other duties as assigned.

Qualifications:
Minimum:

  • Willing to work during or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent of this position includes working in Special Needs or Red Cross Shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • Bachelor's degree from an accredited college or university with a major in health education, community or public health, nursing, one of the biological sciences, education, public relations, communications or one of the behavioral sciences & four years of professional experience in providing health education services; or master's degree from an accredited college or university in one of the areas of study described above and three years of professional experience as described above; or equivalent professional experience.
  • A valid Florida driver's license
